During this symposium, Dr. Dina Moubayed (CHUSJ) and Mr. Dominic Chartrand (nurse, CHUM) defined the transition of care, its issues and impacts, presented the peculiarities of adolescence in the context of chronic illness, and finally identified recommendations and advice to prepare for the transition.
